Overview
Vytalize, a forward-thinking Accountable Care Organization(ACO), sought to increase patient engagement and improve the quality of care.Their goal was to create a streamlined patient intake process that would driveenrollment in care programs while facilitating clinic operations. To do this, they needed a patient-friendly iPad app to simplify check-ins and program enrollment.
Challenge
Vytalize needed a digital solution to replace its manualpatient intake process with a configurable and scalable tool. The primaryobjectives were to:
Drive patient engagement at check-in.
Simplify enrollment in care programs.
Implement COVID-19 screening protocols.
Verify insurance without needing real-time eligibility checks.
However, the app had to fit within a complex multi-practice environment, ensuring smooth integration with clinic workflows and APIs for patient management.
SOLUTIONS
Pi Tech developed a Minimum Viable Product (MVP) of a patient intake iPad app, which included two main components: a patient-facingi Pad app and a clinic-facing web app. The MVP was designed to:
Enable program enrollment
Patients couldeasily access information about Vytalize’s care programs and schedulefollow-ups via SMS.
Streamline patient check-in
The appfacilitated quick check-ins by collecting patient demographic information and seamlesslyintegrating Vytalize’s existing API.
Dynamic Intake Forms
Patients review andsign personalized intake forms that are dynamically generated according to eachprovider's specific requirements.
Capture insurance details
The appallowed patients to upload insurance cards through photos, barcodes, or QRcodes.
The clinic-side web app provided real-time patient status updates, including notifications when a patient checked in and waiting time tracking. Clinic staff could also review completed forms and manage patient flow within the practice.
